A change of circumstance form should be completed if a GPST trainee experiences a change in circumstance that requires them to take time out of the programme (e.g. maternity, paternity, adoption leave). 

On the form, please can you include the following information:

  • Estimated Start Date of leave
  • Estimated End Date of leave
  • Predicted % working (Example: 60% less than full time working) upon return
