A change of circumstance form should be completed if a GPST trainee experiences a change in circumstance that requires them to take time out of the programme (e.g. maternity, paternity, adoption leave).
On the form, please can you include the following information:
- Estimated Start Date of leave
- Estimated End Date of leave
- Predicted % working (Example: 60% less than full time working) upon return
- For information regarding arranging Keeping in Touch (KIT) days, please contact the relevant Lead Employer
- Paternity leave
- If you are returning from maternity/adoption leave and wish to apply for LTFT working please contact the Helpdesk.
